SLHS PIP Program Time Contribution Form
The time contribution listed will be reflected on your quarterly report.
All PIP hours for each month must be submitted by the 10th of the following month to receive credit.
The PIP program will run July 1st through June 30th of each year.
Time contributed by parents or grandparents can be submitted for PIP credit.
Extended family or student help with approval of PIP Coordinator first.
Student hours do not count unless prior approval by the school or PIP Coordinator. |
